Exercise Your Rights Under the Law on Protection of Personal Data (LPPD).

1. General Explanation

As defined in the Law on Protection of Personal Data No. 6698 (“Law”), you, as the “Data Subject,” have the right to make certain requests regarding the processing of your personal data held by MEDSTEIN.

Pursuant to Article 13 of the Law and the “Communiqué on the Principles and Procedures for the Request to Data Controller,” you must submit your requests to our company in writing or by other methods determined by the Personal Data Protection Board.

2. Application Methods

To exercise your rights, you can fill out the MEDSTEIN Data Subject Application Form below and submit it to us using one of the following methods:

• A. Written Application (Wet Signature): You can submit the signed form in person or via a notary public to our headquarters address.
  • Note: The envelope must be marked with “Request for Information under the Law on Protection of Personal Data”.
• B. Registered Electronic Mail (KEP): You can send the form to our registered electronic mail address signed with a secure electronic signature.

• C. Electronic Mail (E-mail): You can send the form to our dedicated privacy email address using your mobile signature/e-signature OR using the email address previously registered in our system.

3. Response Time

Our company will conclude your request free of charge as soon as possible and within 30 days at the latest, depending on the nature of the request. However, if the transaction requires an additional cost, a fee may be charged based on the tariff determined by the Personal Data Protection Board.
